Registration Form

In order to enroll in classes, we require every athlete to have this form filled out prior to instruction. 

For monthly class payment we also have a credit/debit authorization form to put your card on file. South Side Spirit has copies in the office but if you would like to get ahead, please fill this out and bring it to the gym.
